Description
TOTEM-UG is a small size wearable computerized unit suitable to perform Pelvic Floor electrical Stimulation (Pelvic Floor Stimulation) and biofeedback, through vaginal or anal probes. The therapeutic aim is to improve the continence function of the patient exploiting the effectiveness of the perinea stimulation and the great contribution of biofeedback in the functional re-learning. TOTEM-UG, supplied complete with the special software, must be used with a suitable personal computer (either desktop or lap-top) working with a Windows operating system (W-XP or W-Vista or W-7). The connection with the PC is wireless (Bluetooth) so that no wire connection is required. The TOTEM UG unit is applied to the waist of the patient by means of the belt.

EFFECTS OF THE PELVIC FLOOR STIMULATION:
Other than manual re-education and pelvic-perinea physiotherapy, pelvic floor or perinea stimulation is the most popular method for treating incontinence symptoms. Pelvic floor electro stimulation successfully re-stores continence, especially in female patients. Pelvic floor electro stimulation is normally applied using vaginal electrodes (probes), characterized by at least two electrodes, often in the form of rings. The aim is to stimulate the group of anal elevators, especially the pubo-coccygeal muscle. The different effects of electro stimulation, have been observed and demonstrated by many researchers. STRENGTHENING THE PELVIC FLOOR: Electro stimulation improves muscle activity. When the P.C. test (Pubo Coccygeal test) gives a value of 1 or 2 (1= just perceptible contraction, 2= weak contraction), endovaginal electro stimulation has been found very effective, quickly producing an improvement by at least 2 points. This improvement is the result of the electro stimulation providing two separate but concurrent effects: the excitomotor action that increases muscle tone, and the vascular effect improving local circulation. EFFECT ON THE URETHRAL CLOSURE PRESSURE: A significant increase of the urethral closure pressure has been reported by some researchers (Dr. PIGNE’ – OURCIER). EFFECT ON THE VESICAL TONE : A significant improvement of vesicle compliance has been observed, along with a considerable reduction of the non-inhibited contractions of the detrusor.
